An April 4, 2025, complaint investigation found the facility providing ADLs per regulation but cited one deficiency for failure to notify the department of a change in administrator. A July 8, 2025, complaint investigation addressed allegations of failure to ensure food temperature, call light response, resident choice, elopement prevention, and infection control.

Two residents eloped during a fire alarm event, creating an immediate jeopardy situation due to inadequate supervision. The facility was found in compliance with food, call light, and resident choice allegations. One deficiency was cited for failure to provide supervision to prevent elopement; the immediate jeopardy was resolved after corrective actions.
