Inspection Report Summary for The Kensington Sierra Madre
In May 2022 an unannounced inspection of Kensington Sierra Madre was prompted by a complaint that a resident (R1) had been dropped, sustained a pressure injury and had clothing torn during a fall. The evaluator met with Administrator CECILIA DEGRAFF, interviewed staff and R1’s family, and reviewed R1’s medical file. The incident on April 30, 2022 resulted in a fall that was treated by a medication technician who called 911; paramedics transported R1 to a hospital, but no serious injuries were documented. The investigation found no evidence of a pressure injury or of staff misconduct, and the allegations were declared unsubstantiated.
A subsequent visit in April 2024 revisited the same facility on a complaint filed in January 2022. The evaluator, Luis Mora, conducted interviews, reviewed staff rosters, medical records, and observed the environment. The findings confirmed that R1 had sustained unexplained injuries—fractured ribs, wrist and toe injuries—on January 4, 2022, and that staff delayed seeking timely medical evaluation. The facility failed to recognize R1 as a fall risk, did not notify R1’s authorized representative of the injuries, and did not meet basic hygiene needs. Evidence of these deficiencies met the preponderance of evidence standard, resulting in a substantiated report and a civil penalty of $500.00.
The two investigations highlight a pattern of evolving oversight challenges at Kensington Sierra Madre. While the initial review found no substantiating evidence of neglect, the later inspection revealed lapses in timely medical response, risk assessment, and communication with residents’ families. These findings underscore the need for enhanced fall‑risk protocols, prompt medical evaluation, and clear notification procedures to safeguard resident safety and well‑being.
