Westmont of Milpitas, a 740‑type assisted living and memory care facility, underwent a series of unannounced evaluations by the California Department of Social Services Community Care Licensing Division. In March 2021 a technical‑assist visit addressed COVID‑19 infection‑control gaps, recommending clear signage and foot‑operated trash cans. The December 2021 annual inspection, and the subsequent December 2022 inspection, found the facility in compliance with state infection‑control standards: adequate PPE, hand‑washing stations, and a designated isolation area, and it noted that all staff and residents were fully vaccinated.
In March 2023 the department conducted a case‑management review of four reported theft incidents involving residents R1‑R4. The visit confirmed that the facility had reported the incidents to law enforcement and the Ombudsman, had held a town‑hall discussion with residents and families, and had provided lockboxes and security guidance. No deficiencies were cited, but an action plan was discussed to reinforce resident safety and staff training on theft prevention.
The most recent visits in April 2024 shifted focus to medication safety and broader regulatory compliance. An unannounced case‑management visit on April 25 identified a medication error that occurred when a resident administered an extra dose of medication, a situation linked to the resident’s dementia and the presence of her own medication bottle in the room. The analyst issued an advisory note and requested a plan of action from Administrator Gregory Becker to address staff training and medication‑management safeguards.
On April 26 a noncompliance conference involving senior CCLD leadership reviewed the facility’s history of serious violations under Title 22, including personal‑rights, medical‑care, and reappraisal deficiencies. A two‑year monitoring plan and compliance strategies were agreed upon.
