The facility, a three‑story 740‑type assisted‑living home in Thousand Oaks, California, houses 170 residents (150 census). No health‑and‑safety hazards were noted during any of the inspections, and the facility complied with infection‑control protocols, including temperature checks, hand‑sanitizer stations, and adequate PPE supplies. In the annual visit on 24 June 2022, the evaluator praised the cleanliness of the building, the availability of fresh food from Sysco, and the maintenance of common areas and outdoor spaces, issuing no citations.
