No health‑and‑safety hazards were noted during any of the inspections, and the facility complied with infection‑control protocols, including temperature checks, hand‑sanitizer stations, and adequate PPE supplies. In the annual visit on 24 June 2022, the evaluator praised the cleanliness of the building, the availability of fresh food from Sysco, and the maintenance of common areas and outdoor spaces, issuing no citations.

The first unannounced case‑management visit on 26 May 2022 addressed a Report of Suspected Dependent Adult/Elder Abuse filed on 25 May 2022, alleging a violation of personal rights against Resident #1. The evaluator documented a deficiency under Title 22 Section 87468.1(a)(1) concerning personal rights violations and issued a Plan of Corrections with a due date of 30 August 2022.

Across all inspections, the facility demonstrated compliance with environmental and infection‑control standards but exhibited recurring concerns regarding staff‑resident boundary violations. The recorded deficiency underscores the need for stronger policies and training to safeguard resident dignity and uphold California’s personal‑rights requirements.
