Visit Reason
The visit was a follow-up survey conducted on October 20, 2025, to determine if previously cited deficient practices detrimental to resident health, safety, or welfare had been sufficiently corrected as stated in the facility’s approved Plan of Correction.

Findings
The follow-up survey found that the deficient practices leading to a Suspension of Admissions issued on October 11, 2025, were sufficiently corrected to lift the suspension. However, additional deficiencies were cited that require the facility to return to substantial compliance with state laws, rules, and regulations.

Visit Reason
The revisit survey was conducted to assess continued compliance with licensure statutes and regulations after prior deficiencies were found.

Findings
The survey revealed continued violations related to resident safety, daily awareness of resident whereabouts, and failure to fully implement and revise plans of care. These violations were deemed detrimental to the health, safety, and welfare of residents.

Tenn. Comp. R. and Reg. 0720-26-.07(7)(a)(2) was violated as the facility failed to provide safety measures to ensure Resident #5 did not leave the facility without staff knowledge or supervision.
Tenn. Comp. R. and Reg. 0720-26-.07(7)(a)(3) was violated as the facility failed to maintain daily awareness of Resident #5's whereabouts despite a known history of leaving the facility unsupervised.
Tenn. Comp. R. and Reg. 0720-26-.12(5)(a) was violated as the facility failed to fully implement its plan of care and revise it as residents’ needs changed.
