However, the facility lacked an emergency key set, liability insurance, and the water heater temperature (96 °F) was below the mandated 105 – 120 °F range. The inspector recommended that these items be rectified before the facility could be licensed. Component III orientation was waived due to the administrator’s experience.

The 25 August 2023 audit, the required first annual inspection, uncovered extensive non‑compliance. The facility housed five residents but employed no staff other than the administrator, and the kitchen lacked proper labeling and separation of resident and staff food. Disinfectants, medications, and sharp objects were unsecured, and the hot water temperature was recorded at an unsafe 150 °F. The garage refrigerator and freezer were dirty, trash bins were full, and overall sanitation was poor, creating a health and safety risk. Additionally, the facility failed to secure disinfectants, provide CPR/first‑aid training, and develop resident participation plans. The inspector documented a series of Type A and Type B deficiencies with correction plans due by 26 August 2023 (and 1 September 2023 for resident participation). Failure to comply could trigger civil penalties.
