The complaint investigation was completed on or about May 10, 2023, substantiating that the facility failed to provide safety to one resident who walked out of the facility through an unsecured door with a disabled alarm.
Findings
The investigation found that Resident #1 exited the facility unnoticed for approximately 21 hours and walked 10 miles to an independent facility. The resident was returned without injury and transferred to the Memory Care Unit. The facility violated safety regulations by disabling the door alarm and failing to ensure resident safety.
