Visit Reason
Commission surveyors conducted a Life Safety survey at The Village at Bellevue Assisted Care Living Facility on or about October 14, 2024.
Findings
The facility failed to produce documented evacuation plans for all residents and did not maintain working smoke alarms in multiple resident rooms. Additionally, the facility lacked documentation of required quarterly fire drills for each work shift and failed to conduct fire drills during sleeping hours as required.
