The Department of Social Services conducted a series of unannounced inspections at Village at Sherman Oaks (Facility #197608694) during 2021, 2022, and 2023. Two case‑management visits in 2021 addressed self‑reported incidents involving Resident #1. On 26 October 2021, a resident was heard yelling in the dining room while a staff member shouted “Shut up”; the 11/04/2021 visit confirmed no immediate health or safety concerns but required further investigation. A second incident, reported on 13 November 2021 and investigated on 12/14/2021, likewise involved Resident #1 and was found to present no immediate risks, though the inspector noted additional review was needed. Infection‑control signage, temperature checks, and weekly COVID‑19 testing for un‑vaccinated staff were all in place. The 7 February 2023 visit, also conducted by Balisi and met with Director of Assisted Living Ada Navarrete, reviewed Building A (1st‑3rd floors) and Building B (2nd floor). Observations mirrored the 2022 findings: well‑maintained bedrooms, common areas, kitchen, and memory‑care rooms; comprehensive PPE storage; and adequate emergency food supplies. No violations or safety concerns were documented during either inspection.
Across the four reports, Village at Sherman Oaks consistently met regulatory expectations for environmental health, resident accommodations, and infection‑control protocols. While the 2021 case‑management visits highlighted isolated resident‑staff incidents, they did not reveal systemic deficiencies. The facility’s census rose from 67–61 residents in 2021 to 87 in 2023, and capacity increased from 100 to 179, yet the inspection findings remained positive, indicating effective oversight and compliance management throughout the period.
