Visit Reason
A complaint survey was conducted on January 10, 2019, following reports of a bed bug infestation at the Crown Cypress Assisted-Care Living Facility.
Complaint Details
The complaint survey was substantiated with findings of live bed bugs and failure to control infestation.
Findings
Surveyors observed live bed bugs in the facility and found that the facility failed to prohibit an infestation of bed bugs. The facility was assessed a civil monetary penalty for violating the rule requiring a safe, clean, and sanitary environment free of insects and vermin.
