Visit Reason
Post licensing unannounced inspection visit to evaluate the facility for licensing compliance.
Findings
The facility was toured and evaluated for compliance with regulations. Several deficiencies were cited including issues with criminal record clearance for staff, improper use of common areas, incomplete medication records, insufficient dementia training for staff, and lack of proof of first aid training.

CCR 87355(c): Criminal record clearance for staff #2 was not associated with the facility, posing an immediate health or safety risk. This deficiency was corrected during the inspection.
CCR 87307(a): Staff used the living room sofa for sleeping at night, failing to reserve common areas for common use, posing a potential health and safety risk.
CCR 87465(h)(6): Six prescription and OTC medications for client #2 and OTC Robitussin for client #1 were not recorded on centrally stored medication records, posing a potential risk.
HSC 1569.626: Staff #4 has only received 5 hours of required 12 hours dementia care training after over 4 weeks of employment, posing a potential risk to clients.

Visit Reason
The visit was a prelicensing inspection for Unified Hearts Home Care LLC applying for RCFE licensure to serve 6 elderly clients, including 5 non-ambulatory and one bedridden, in 6 private rooms.
Findings
The facility was toured and found to have appropriate rooms, emergency systems, and safety equipment, but several items must be addressed prior to licensure including updating the Emergency Disaster Plan, fitting beds with mattress pads, establishing telephone service, and obtaining an internet-capable device for client use.
Deficiencies (4)
Emergency Disaster Plan must be updated to include corrected location of water shut off and fire extinguisher locations as required by Section 87212.
Beds are not fitted with mattress pads as required by Section 87307 Personal Accommodations and Services.
There is no telephone service established as required by Section 87311 Telephones.
An internet-capable device for client use has not yet been obtained as required by Health & Safety Code 1569.319.
