Inspection Report Summary for Vi at Palo Alto

The California Department of Social Services, Community Care Licensing Division, conducted a series of unannounced evaluations at the VI AT Palo Alto facility (Facility # 435200930) between June 2021 and April 2023. Early visits focused on compliance with COVID‑19 protocols and routine case‑management oversight. In each of those inspections the agency noted that the facility had established screening stations, hand‑washing signage, adequate personal‑protective‑equipment supplies, and appropriate food storage, and no deficiencies were cited.

In December 2021 the facility reported the death of a resident (R1). A subsequent case‑management visit collected documentation and confirmed that other residents were receiving meals and medication assistance. No regulatory violations were identified. In January 2022 a tele‑visit addressed the facility’s COVID‑19 status, noting nine infected staff members and zero infected residents. The agency offered additional recommendations for distancing signs and hand‑washing reminders, but again found no deficiencies.

The first regulatory deficiency emerged in November 2022 when a self‑reported incident involving staff member S1 and resident R1 prompted a detailed investigation. The internal report documented that S1 had allowed R1 to remain in a soiled state and had been distracted by television viewing. The agency determined that the facility had not provided sufficient staff support for R1’s dressing needs, a violation of residents’ personal rights. A deficiency was cited under the California Code of Regulations (Section 87468.2(a)(4)), with a plan of correction deadline of 11 24 2022. The agency noted that staff training addressing cleanliness and supervision had already occurred on 11 2 2022, and the deficiency was marked as cleared during the visit.

A second incident, reported in March 2023, involved a staff member alleged to have physically abused R1. The internal investigation documented that S1 was assisting R1 at approximately 3:30 AM on 3 26 2023, redirected R1 to bed, and that R1 had reported a hit to another staff member. A wellness check found no injuries. While the incident was noted as an unusual event, the agency did not cite a regulatory deficiency, and the investigation concluded that no violations had occurred.

Overall, the facility has maintained compliance with general health‑and‑safety standards and has addressed specific deficiencies through staff training and corrective plans. The pattern of visits demonstrates a responsive oversight framework that promptly identifies and remediates care‑related lapses while affirming the facility’s adherence to core regulatory requirements.
