The inspection was conducted as a complaint investigation based on allegations of sexual and psychosocial abuse, physical and verbal abuse, involuntary seclusion, untimely assistance with activities of daily living, medication administration errors, over sedation, and provision of palatable meals.

The complaint investigations involved allegations that the facility failed to ensure residents were not sexually or psychosocially abused, physically or verbally abused, involuntarily secluded, failed to provide timely assistance with activities of daily living, failed to administer medications according to physician orders, failed to ensure residents were not over sedated, and failed to provide palatable meals. The investigation included observations, interviews, and record reviews. The facility was found to have failed to report an incident of unwanted sexual advances between residents to the state agency.

The inspection was conducted as a complaint investigation based on allegations that the facility failed to ensure proper infection control procedures regarding COVID, failed to ensure residents were not violated, and failed to have and/or implement an effective pest control program.

The investigation found no deficiencies; no COVID positive residents were present, no residents were on lockdown or quarantine, and no pests were observed. Resident records, facility documents, and staff interviews supported compliance with policies and procedures.

Complaint investigation #OK00060259 was conducted with allegations related to infection control, resident rights violations, and pest control. The investigation was unannounced and included observations, interviews, and record reviews. No deficient practice was cited.
