Sommerset maintains a strong compliance record with improvement following a single deficiency cluster. No fines, immediate jeopardy findings, or enforcement actions have been imposed. Renewal license effective November 30, 2025 through November 30, 2028.

The 126-bed facility comprises studio, suite, and one-bedroom units in a single-story building. Personalized assistance with activities of daily living, medication management, housekeeping, laundry, and chef-prepared meals with attention to dietary preferences and restrictions are provided. The specialized 20-bed Alzheimer’s and dementia care unit offers staff trained in behavioral management, communication techniques, cognitive stimulation, and therapeutic activities with 2 hours of structured programming daily. No fines, immediate jeopardy findings, or license suspensions have been imposed. A renewal license was issued February 4, 2025, effective May 1, 2025.

The 110-bed facility comprises one-bedroom apartments in a two-story building with 545 square feet per unit. Residents furnish with personal belongings. Twenty-five full-time staff provide support. Services include personalized assistance with meals, medication management, personal care, housekeeping, transportation, and recreational activities. All-inclusive room and board covers rent, utilities, maintenance, and meals. Fitness and recreation support resident engagement. Current occupancy is 73% (80 of 110 beds), reflecting recent market demand decline. Walk Score 67 indicates moderate walkability; facility is 5 miles from Oklahoma City center. BBB Rating: A+ (Not Accredited). The facility participates in Oklahoma’s ADvantage Waiver and Personal Care Program for Medicaid-eligible residents.

The April 2025 deficiency involved procedural failure to report to state authorities rather than substantiated allegations.
