The Bureau of Assisted Living, Southern Regional Office, conducted a verification visit at Vista Pointe, a community-based residential facility (CBRF), to verify correction of previous deficiencies.

Findings
As a result of the survey, zero deficiencies were identified and all previous deficiencies from the Statement of Deficiency dated 09/18/2025 were corrected.

Visit Reason
On 09/17/2025, the Bureau of Assisted Living, Southern Regional Office, conducted a complaint investigation and abbreviated licensing survey at Vista Pointe, a community-based residential facility in Menomonee Falls, WI.

Complaint Details
The complaint was unsubstantiated. The investigation included review of text messages and interviews regarding allegations of neglect related to medication administration and resident care. Surveyor found no evidence that misconduct could be proven and the incident was not reportable.
Findings
As a result of the survey, 5 deficiencies were identified. The complaint was unsubstantiated. Deficiencies included failure to maintain documentation of investigations and reports of employee misconduct, failure to make employee records available upon request, inadequate heating system maintenance, and incomplete fire and emergency evacuation drills.
