Visit Reason
The inspection was a Life Safety survey conducted by Commission surveyors at Walking Horse Meadows Assisted Care Living Facility on or about December 16, 2024.

Findings

Commission surveyors conducted a Life Safety survey at AHC Cumberland Skilled Nursing Facility from September 5, 2024, through November 7, 2024.

The facility was found to have multiple violations including failure to provide appropriate fiscal resources and personnel, inadequate treatment and monitoring of residents, failure to report abuse and neglect timely, and failure to maintain proper documentation for transfers and care.
