The revisit survey was conducted to assess continued violations of licensure statutes and regulations at Wellington Manor Jackson Assisted Care Living Facility, which were found to be detrimental to the health, safety, or welfare of residents.

Findings
The survey revealed deficiencies including insufficient staffing leading to a resident eloping, failure to pay vendor invoices causing service interruptions, broken safety gate allowing unsupervised resident exit, and failure to fully implement the plan of correction or revise care plans as residents' needs changed. These violations resulted in suspension of admissions and assessment of four Type A Civil Monetary Penalties totaling $6,000.
