Findings
The facility had multiple violations including failure to assess residents or follow service plans, improper food handling, missing authorized electronic monitoring records, and noncompliance with Life Safety Code standards including emergency plans and fire alarm visibility.

The facility failed to procure food from acceptable sources, or failed to handle food, subject to spoilage, as required.
The facility failed to obtain and maintain a properly completed Request for Authorized Electronic Monitoring in the clinical record.
The facility failed to ensure the building and structure complied with the 2000 edition of the Life Safety Code, NFPA 101.
