The 2021 inspections of Wind Chime of Marin found the facility operating in compliance with California’s Title 22 standards. During the unannounced annual visit on June 25, 2021, LPAs Farhaan Sarangi and Erik Gonzalez‑Campos noted a clean environment, proper food storage, and adequate PPE. Staff were reminded to wear masks and gloves, and no deficiencies were cited. The September 17, 2021 case‑management inspection addressed a resident‑staff incident that had been self‑reported. The facility had a mitigation plan approved in 2021 and maintained an adequate PPE supply, but the deficiencies prompted a plan of correction and required documentation of staff training, updated emergency plans, and proof of liability insurance. The LPA also noted that fire‑extinguishers and the sprinkler system were in service and that disaster drills had been conducted quarterly.
Across the three visits, a pattern emerges: initial compliance with basic hygiene and licensing requirements gave way to emerging safety risks related to resident self‑care environments and regulatory documentation.
