Visit Reason
The Department conducted an annual licensure survey at Ridgemont Manor #2 on or about August 10, 2018.
Findings
The facility failed to conduct timely resident assessments and develop care plans within required timeframes. Medication storage was unsafe with unattended cabinets and improperly stored food. The facility lacked a performance improvement program and failed to meet dietary service requirements including special diets and menu planning.
