Visit Reason
The Department conducted an annual licensure survey at Ridgemont Manor #2 on or about August 10, 2018.

Findings
The facility failed to conduct timely resident assessments and care plans, lacked a performance improvement program, improperly stored medications and food, and failed to meet dietary needs and orders for residents. Several violations of state rules and regulations were documented.
