Oak Ridge Health and Rehabilitation’s inspection record is mixed, with repeated deficiencies in food safety, resident care, and safety protocols across recent inspections. On September 26, 2024, inspectors cited three deficiencies including unlocked doors on the Memory Unit and unsanitary kitchen conditions, while the November 3, 2023 annual survey found eight deficiencies involving medication management, privacy violations, and dietary violations. A September 13, 2023 complaint investigation identified immediate jeopardy when a resident eloped due to a malfunctioning security system and failure to report the incident properly.
