BROOKDALE TYLER SOUTH underwent a comprehensive inspection on March 13, 2024, resulting in six state standard violations. All of these deficiencies were classified under the Life Safety Code, indicating no Health Code violations were cited during this inspection. The facility's issues fell into several themes, including fire safety and building maintenance. Specifically, the facility failed to comply with Chapter 19 of NFPA 101, ensure the building and structure met applicable chapters of the Life Safety Code, and maintain a required sprinkler system in accordance with NFPA 25. Additionally, hazardous areas were not constructed in compliance with licensing standards for assisted living facilities, and an emergency generator was not in compliance. These issues were largely corrected between February 20, 2019, and April 30, 2019. No enforcement actions were found related to this inspection.
