Additionally, two residents lacked up‑to‑date TB test or chest‑x‑ray results, and one resident’s medication orders were missing discontinuation directives. Perhaps most concerning, a staff member’s fingerprint clearance was incomplete; the individual was removed from the facility during the inspection and a civil penalty of $500 was assessed.

The inspection report required multiple corrective actions: obtaining missing training certificates and TB test results by 15 December 2023, securing fingerprint clearance for the staff member by 22 November 2023, and addressing the other documented deficiencies within their specified timelines. While the facility demonstrates capacity to address specific deficiencies when identified, the frequency of documentation lapses indicates a need for stronger internal oversight and routine compliance checks to ensure sustained resident safety and regulatory adherence.
