May 29, 2025 biennial survey found 6 deficiencies including incomplete background checks, missing resident service plan documentation, incomplete progress notes, and improper storage of poisonous materials.
Three of four staff lacked required Department-approved certification training within 120 days of hire, and personnel files missed job descriptions for all four staff reviewed.
Complaint investigations on February 5 and March 1, 2024 found no deficiencies and confirmed substantial compliance with licensing and care regulations.
