March 2024 complaint investigation substantiated one deficiency involving inadequate supervision during showering resulting in a resident fall. Most other complaint investigations were unsubstantiated. No fines, immediate jeopardy findings, or license suspensions documented.
Improvement trend evident: 2020-2024 largely compliant with isolated deficiencies; recent 2025 findings more focused on administrative and medication management protocols.
Dogwood Creek provides 10 amenities: one/two-bedroom suites, three daily meals with individualized dietary accommodation, kitchenette in units, weekly maid service, 24-hour staffing, smoke detectors, emergency call buttons, full recreational/cultural/social programming, and wooded residential setting.
