A January 15, 2020, relicensure survey cited four deficiencies: failure to ensure five of eight sampled residents did not exceed the care level licensed to provide, failure to ensure registered nurse supervision of self-administration of medications for one sampled resident, failure to maintain organized and accurate clinical records, and failure to ensure third party staff providing sitter services had current credentials and background checks.

These deficiencies represented potential for more than minimal harm. Relicensure surveys on April 30, 2020, April 20, 2021, and April 12, 2022, found no deficiencies. A March 31, 2023, complaint investigation substantiated one deficiency related to infection prevention and control and cited two additional deficiencies: failure to maintain the kitchen in accordance with food service regulations including cleanliness of ovens, stove, grill, drawers, can opener, window blinds, and sealing of frozen food; failure to ensure potentially hazardous foods were disposed of within 24 hours; and failure to ensure blood pressure cuff was disinfected between residents during medication administration. A April 1, 2025, license renewal authorized continued operation with a maximum capacity of 30 beds.

A November 4, 2025, relicensure survey found no deficiencies. An November 17, 2025, initial licensing inspection for Alzheimer’s dementia special care designation found no deficiencies. The facility’s occupancy has declined from 21 residents in January 2020 to 16 residents in November 2025. No fines or license suspensions are recorded.

The facility specializes in Alzheimer’s and dementia care with all 30 beds designated for cognitive support. Licensed capacity is 30 beds with current occupancy of 16 residents (53 percent). Occupancy has declined significantly. Nursing hours 28 percent above Oklahoma benchmark.

Relicensure surveys from 2021 through November 2025 finding no deficiencies indicate improved compliance.
