A revisit on January 30, 2020, confirmed correction. A July 29, 2020, COVID-19 special focus infection control survey found no deficiencies. Relicensure surveys on August 18, 2020, November 10, 2022, and June 20, 2025, found no deficiencies.

A February 28, 2024, licensure survey with complaint investigation found one deficiency: the facility failed to report required incidents to the Oklahoma State Department of Health within one business day for five of twelve sampled residents. The Administrator acknowledged late reporting and lack of a reporting policy.

The facility’s occupancy declined from 51 residents in October 2019 to 47 residents in February 2024. No fines or license suspensions are recorded.
