A May 25, 2023, complaint and state licensure survey identified six deficiencies spanning dietary staff food service training, incomplete annual comprehensive assessments, absent RN or physician signatures on assessments, failure to conduct personal interviews, improper transcription of physician orders by unqualified staff, and failure to identify close contacts for COVID-19 positive residents. The facility submitted a plan of correction with a target completion date of July 19, 2023. A July 23, 2024, complaint investigation addressing notification of residents’ representatives and hospice, care coordination, advance directives, and rate increase notifications found no deficiencies.

An October 31, 2024, complaint investigation regarding pet vaccination and care found no deficiencies. A July 20, 2025, relicensure survey found no deficiencies.

The pattern reflects deficiency clusters in early years followed by corrective responses and zero deficiencies in recent inspections. No fines or license suspensions are recorded. Current occupancy at 43 percent is lower than state average, suggesting reduced demand.
