A June 12, 2024, relicensure survey identified one deficiency: failure to ensure comprehensive assessments included personal interviews between residents or their representatives and staff for seven of eight sampled residents. This deficiency represented potential for more than minimal harm. A November 10, 2025, relicensure survey and complaint investigations found no deficiencies and substantiated no complaints related to abuse or communication access. The facility’s occupancy has grown steadily from 31 residents in July 2020 to 47 residents in November 2025.
