All reviews concluded with no deficiencies cited under Title 22, Division 6, Chapter 8 of the California Code of Regulations. The facility’s administrator certificates were up‑to‑date, and the LPAs documented that the center remained in good standing with no safety hazards detected.

The March and September 2022 visits shifted to case‑management investigations concerning Resident #1 (R1). The incident reports described a car accident on September 15, 2022, and an earlier allegation of financial exploitation. LPAs gathered extensive documentation—physician reports, preplacement appraisals, power‑of‑attorney filings, and police statements—to assess R1’s capacity and the adequacy of the facility’s protective measures. Findings indicated that R1, while exhibiting mild cognitive impairment and occasional confusion, is generally able to leave the facility unassisted under supervision. Importantly, the investigations revealed no violations of Title 22 regulations, and the facility was actively collaborating with Adult Protective Services, primary care providers, and law enforcement to safeguard R1’s physical and financial well‑being.

Across all visits, Greenhaven Place demonstrated consistent compliance with CCLD requirements, maintained a safe environment for residents, and responded appropriately to incident reports. The pattern of unannounced inspections and the absence of cited deficiencies reflect a facility that adheres to state regulations and prioritizes resident safety and quality of care.
