Inspection Report Summary for Teresian House Center for the Elderly

During the reporting period the facility received a total of 23 citations, comprising 18 standard health deficiencies and 5 Life Safety Code deficiencies. Six inspections resulted in citations while eight inspections yielded no citations. One citation involved actual harm or immediate jeopardy to resident health or safety, representing 4.0 % of all citations. The overall citation rate per 100 occupied beds was 2.4, and the facility’s citation count exceeded the state average of 31.

The November 8, 2024 certification/complaint survey produced the largest number of cited items, with eleven citations in total. Standard health deficiencies included failures in activities of daily living and maintenance of abilities, inadequate care plan timing and revision, lack of comprehensive assessment after significant change, improper food procurement and preparation, medication error rates exceeding five percent, inadequate reporting of alleged violations, and deficiencies in respiratory/tracheostomy care and suctioning. This inspection highlighted critical gaps in both resident care and environmental safety.
