Hot water temperatures in several rooms exceeded Title 22 limits (up to 124 °F) or fell below the required minimum (as low as 68 °F in a bathroom). Physical plant deficiencies included a loose shower fixture, broken wall corner, malfunctioning kitchen drawer, and a deteriorated wooden fence. The inspection noted that the facility’s medication room was properly secured, and that infection‑control signage was adequate. The licensing agency issued a Plan of Correction with a 12‑day due date for the hot‑water and structural issues, and a one‑month deadline for repairing the fence. Failure to correct would invite civil penalties.

In the 2023 inspection, the facility again met infection‑control expectations, with visible COVID‑19 signage, visitor screening, staff mask use, and adequate hand‑sanitizer supplies. A single maintenance deficiency was identified: a loose shower fixture in room 122. The agency provided a short‑term Plan of Correction with a 7‑day deadline for the fix. No new compliance gaps emerged beyond the physical‑plant maintenance item, and the facility continued to maintain adequate perishable and non‑perishable food supplies for two and seven days, respectively.
