Interviews with staff and residents, coupled with a review of incident logs, revealed that the resident in question had left the facility on May 1 and had not returned, negating the fall and injury allegations. Records also showed pendants had been ordered but were on back‑order; the administrator had secured replacements from an alternate location. However, the evaluator documented 153 instances in which staff took more than an hour to respond to pendant calls over an 18‑day period. This finding was substantiated, citing a Title 22 deficiency under Personnel Requirements‑General. The facility was required to submit a staffing plan to ensure timely response to resident calls.

A September visit examined claims that staff failed to communicate with residents’ responsible parties. Interviews with residents, staff, and the responsible parties, alongside a review of communication logs, confirmed that the facility contacted emergency contacts for medication changes and significant condition shifts. The evaluator concluded the allegation was unfounded.

The pattern emerging from these investigations shows that while most complaints were dismissed as unsubstantiated or unfounded, a significant safety concern—delayed response to call pendants—was verified. This substantiated finding underscores an immediate risk to resident safety and highlights the need for a staffing adjustment to meet California’s regulatory requirements for timely call‑button response.
