Inspection Report Summary for Ivy Park at La Jolla
Ivy Park at La Jolla, a 740‑type facility with a licensed capacity of 76 residents, underwent a series of inspections and a case‑management visit between August 2024 and August 2025. In the pre‑licensing visit on August 6, 2024, Licensing Program Analyst Daniel Pena observed the facility during a transition of ownership. The fire clearance had been granted on May 6, 2024, but the analyst noted that further evaluation by the fire inspector and the Centralized Application Bureau was required before the pre‑licensing process could be finalized. The administrator, Megan Franz, received the report and appeal rights, confirming receipt of the documentation.
A subsequent pre‑licensing and Component III inspection on August 14, 2024, confirmed that the facility met all applicable California Code of Regulations requirements. The analyst verified that the administrative organization form reflected the change of ownership, that liability insurance was current, and that resident capacity and waiver designations matched the facility’s license. An on‑site tour demonstrated compliant physical plant conditions, sanitary resident rooms, functioning smoke and carbon monoxide detectors, adequate food storage, and proper medication handling. No deficiencies were cited, and the facility’s application was noted as pending final review.
On September 17, 2024, a case‑management incident visit addressed a self‑reported event involving Resident #1 who exited the facility unassisted through delayed egress doors on September 13. Staff located the resident unharmed at a nearby store and escorted them back to the facility. The licensing analyst, Juliana Barfield, requested relevant records and found no deficiencies; the incident was documented and the resident’s safety was affirmed. Finally, on August 7, 2025, an unannounced required annual inspection by Licensing Program Analyst Ramin Hashemi reviewed the facility’s records, physical condition, staffing qualifications, and compliance with hospice and bedridden waiver provisions. The facility, housing 54 residents at the time, met all health, safety, and operational standards, with no deficiencies noted. Across all visits, the recurring theme was that Ivy Park at La Jolla remained in regulatory compliance, maintained clean and safe living spaces, and demonstrated responsiveness to incident reporting and licensing requirements.
