Inspection Report Summary for Oakmont of Burlingame

Oakmont of Burlingame underwent an unannounced pre‑licensing visit on June 27 2024. Licensing Program Analyst Grace Donato met with Executive Director Oreisha Morgan and toured the facility, noting clear passageways, adequate linens, functioning carbon‑monoxide monitors, and properly maintained fire extinguishers scheduled for service on July 5. The resident bathrooms were in good repair with grab bars and non‑skid flooring, and the kitchen stock and medication audits met Title 22 requirements. No deficiencies were cited and the report confirmed compliance with all regulatory standards.

A follow‑up Case Management visit on June 5 2025, conducted by LPA Murial Han, addressed an incident reported by the facility. On May 26, 2025 a resident in the Memory Care unit (R1) was escorted to a first‑floor celebration and later left the premises unassisted when the caregiver attended another resident. During the June visit the director explained the event, the LPA reviewed documentation, and no deficiency was cited. The facility was reminded of its responsibilities and a copy of the report was provided.

The subsequent Case Management visit on June 18 2025, again led by Murial Han, re‑examined the same incident with Director of Sales Antonio Leon and Health Services Director Cathy Nugyen. The LPA learned that the supervising caregiver had been attending to two other residents, creating a lapse that allowed R1 to exit the community unobserved. Because R1, diagnosed with moderate late‑onset Alzheimer’s dementia, is not permitted to leave unattended, the LPA cited a deficiency under California Health and Safety Code LIC 809D. The facility was informed that failure to correct the deficiency could result in civil penalties and was given an opportunity to submit a plan of correction.

Across these visits a consistent theme emerges: Oakmont’s environmental and procedural controls are generally sound, yet supervision gaps in the Memory Care unit during high‑traffic events have led to safety concerns. The first follow‑up found no immediate violations, but the second visit identified a substantive deficiency, underscoring the need for stronger staffing protocols and door‑control measures to protect residents who wander. The agency’s documentation reflects a careful, step‑by‑step approach to identifying, notifying, and correcting non‑compliance while preserving residents’ rights and safety.
