The most recent inspection on February 11, 2025, was a complaint investigation that found no deficiencies and the complaint was unsubstantiated. Earlier inspections showed a mix of results, with a follow-up visit in January 2024 confirming correction of prior deficiencies and an August 2023 survey citing several deficiencies related mainly to medication administration and updating individual service plans. That August 2023 inspection also included a $2,400 forfeiture and required corrective actions and staff training to address those issues. Complaint investigations were mostly unsubstantiated except for one substantiated complaint in August 2023 concerning medication errors and service plan updates.
