The California Department of Social Services’ Community Care Licensing Division first visited COGIR of Brentwood on October 20, 2022 to complete a Component II compliance call. The most recent unannounced visit, December 14, 2023, addressed an incident in which a resident suffered an unwitnessed fall during a recreational activity, resulting in a hip fracture and subsequent surgery. The facility’s Health and Wellness Director confirmed the resident was now in a skilled nursing facility recovering and that a follow‑up evaluation would be conducted upon return.
