On-site annual compliance inspection of an Assisted Living Home conducted on November 20, 2025.
Findings

On-site annual compliance inspection of an Assisted Living Home conducted on June 30, 2023.
Findings
The inspection found three deficiencies related to service plan updates, means of egress alerting, and securing oxygen containers. Plans of correction were provided for all deficiencies.

The manager failed to ensure a resident had a written service plan reviewed and updated within 14 calendar days after a significant change in the resident's physical health condition and services.
F. A manager of an assisted living facility authorized to provide directed care services failed to ensure the means of exiting the facility controlled or alerted employees of a resident's egress, posing a risk of unawareness of the resident's whereabouts.
The manager failed to ensure oxygen containers were secured in an upright position, as two oxygen cylinders were observed unsecured in a resident bedroom closet.
