Visit Reason
On-site compliance (annual) inspection of AZ LIFE ASSISTED LIVING HOME LLC conducted on June 10, 2025.
Findings
Two deficiencies were found related to resident care authorization and controlled substances inventory. The facility failed to obtain required medical practitioner determinations for a non-ambulatory resident and did not maintain proper inventory records for controlled substances.
Deficiencies (2)
R9-10-815 — The manager failed to ensure the facility did not retain a resident confined to a bed or chair due to inability to ambulate without obtaining a signed and dated determination from a primary care provider at onset and every six months thereafter confirming the resident's needs could be met by the facility.
R9-10-816 — The manager failed to implement policies and procedures for inventorying controlled substances, resulting in lack of documentation for Oxycodone and Lorazepam medications on site.
