  • BROOKDALE SHADOWLAKE underwent its most recent comprehensive inspection on September 21, 2023, resulting in six state standard violations. The Health Code deficiencies consisted of a single issue related to food storage, serving, and reheating temperatures, which was corrected by September 23, 2023. In contrast, the Life Safety Code had five instances of non-compliance, all stemming from a series of issues identified during an inspection on September 3, 2019. These included inadequate fire drills, a malfunctioning fire alarm system, and insufficient sprinkler maintenance. All of these deficiencies were corrected by October 15, 2019. No enforcement actions were found in relation to this facility.
