The following deficiencies were found during the on-site compliance inspection and investigation of complaint 00103083 conducted on August 28, 2025.
Findings
The inspection found four deficiencies related to staff training on fall prevention and tuberculosis, resident tuberculosis screening, and the functionality of an exit door alarm. Plans of correction were provided for all deficiencies.
