October 18, 2022 investigation substantiated medication errors causing a resident to be sent to the ER and failure to provide proper medication training and care.
March 31, 2022 complaint inspection cited five deficiencies including no fire drills in 2021, inadequate staff training, fire safety violations, and poor maintenance of ceilings, floors, and medication records.

This facility’s common areas are very dirty, outdated, and unkept. The elevator is always broken. And the food is not healthy at all. And the care???? You MUST check up behind them and stay on them to ensure things are done—and done right! We had to install cameras in my mother’s apartment to keep watch! The apartments are OK inside, depending on what floor it’s on, but it’s everything else that is raggedy! They will sell it to you, though, like it’s a nice place—but the staff are very unorganized, overworked, and can be rude! And the Director, Deb?!? Not professional in her demeanor at all! Even the EMT that came to see about my mother after falling said, “I would never allow my family to stay here! It’s unsafe and they don’t care!” I removed my mother as fast as I could—but she died that same week, smh… Please don’t take your loved ones here! My mother died EARLIER than doctors expected because of this place and lack of quality care!What do you like most about living here? Answer: Nothing Are there enough caregivers to handle the residents' needs, especially during emergencies? Answer: No Are the common areas pleasant and comfortable for socializing or relaxing? Answer: No How would you rate the quality and variety of the meals? (1-5) Answer: 2 Are special diets handled well? Answer: No Is the dining schedule flexible enough to meet your preferences? Answer: No How do you feel about the level of social interaction available with other residents? Answer: If your loved one can take themselves then yes but if they are not mobile NO How was the units offered in this community? Answer: N/a Do you feel safe living in this community? Answer: No How well do you think the community handles emergencies or unexpected events? Answer: Not good at all They left my mother on the floor after a fall for 2 hours! Are there any hidden fees or unexpected charges? Answer: No Would you recommend this community to others looking for senior living options? Answer: Absolutely not Is there anything you wish you knew before moving in? Answer: I wish I knew EVERYTHING! She would have never been there! Died EARLY because of the lack of care!
