The inspection was conducted to investigate regulatory insufficiencies related to incidents and complaints involving tenant care and safety at Edencrest at Kettlestone Memory Care.

Complaint Details
The investigation was triggered by complaints #128788-C and #129203-C and incidents 128141-I and 130026-I. The complaints were substantiated by findings of failure to perform required safety checks and inadequate tenant care.
Findings
The facility failed to provide adequate and appropriate care to two former tenants, Tenant C1 and Tenant C2, by not performing required hourly safety checks, resulting in a fall and an elopement incident. Staff responsible were suspended and did not return to work.

Staff failed to perform required hourly safety checks, resulting in one tenant being left on the floor after a fall and another tenant eloping by removing window blocks and lowering himself from a second-story window.
