Complaint Details
The following deficiencies were found during the on-site compliance inspection and investigation of complaint AZ00190299 conducted on September 21, 2023.
Findings
The inspection found three deficiencies related to failure to provide assisted living services as per service plans, incomplete documentation of services provided, and incomplete service plan content regarding cognitive stimulation and meal encouragement.
Deficiencies (3)
The manager failed to ensure a caregiver provided a resident with the assisted living services in the resident's service plan, as two of four residents did not receive the expected personal care or bathing services as documented.
The manager failed to ensure a resident's medical record contained documentation of assisted living services provided, as oral care services were not documented for one resident on multiple dates.
R9-10-808 — The manager failed to ensure a service plan included cognitive stimulation and activities to maximize functioning and encouragement to eat meals and snacks for one resident receiving directed care services.
