A.R.S. § 36-420.04.C — The facility failed to maintain a standardized emergency responder form for each resident including key contact information, medication services, and basic medical history for three residents reviewed.

R9-10-803 — The manager failed to ensure a resident's health and safety was protected when a resident fell and hit their head during transport with a Hoyer lift, resulting in hospitalization and stitches.

R9-10-819 — The disaster plan did not include when and where residents would be relocated during a disaster, posing a risk to resident health and safety.

R9-10-819 — The manager failed to ensure disaster drills were conducted on each shift at least quarterly and documented, with no drills after October 9, 2025, available for review.

R9-10-820 — The manager failed to ensure pets were licensed consistent with local ordinances; a dog living at the facility had no current Maricopa County license documentation.

The manager failed to ensure a caregiver provided documentation of completion of a caregiver training program approved by the Department or the Board of Examiners for Nursing Care Institution Administrators and Assisted Living Facility Managers. This posed a risk as the individual hired as a caregiver was not qualified to provide required services.

The administrator failed to ensure sufficient personnel with the qualifications, experience, skills, and knowledge necessary to meet the needs of a resident and ensure their health and safety were present on the premises. This posed a risk as a qualified personnel member was not available to meet resident needs.
