Visit Reason
The Office of the State Fire Marshal conducted a scheduled fire safety inspection at the facility.

Findings
The inspection found that sprinkler systems were tested and maintained with replacements made, and new hydraulic design information placards were provided. Fire drills were completed as required, resulting in an overall approved status.

Deficiencies (3)
IFC 903.5 (2021) - Sprinkler systems shall be tested and maintained in accordance with Section 901. Sprinklers were replaced as required.
25.5* Hydraulic Design Information Sign - The installing contractor shall identify a hydraulically designed sprinkler system with a permanently marked weatherproof sign. New placards were provided.
Fire Drills - At least twelve planned and unannounced fire drills shall be held every year. Drills were completed as required.

Visit Reason
The Department completed a full inspection of the Assisted Living Facility to determine compliance with Assisted Living Facility requirements.

Findings
The facility was found not to meet the Assisted Living Facility requirements due to deficiencies related to assessment documentation and safety assessments. The facility has begun correcting the deficiencies and updated their processes to meet assessment timeline requirements.

Deficiencies (2)
WAC 388-78A-2090 Full assessment topics. The assisted living facility must obtain sufficient information to assess each resident's capabilities, needs, and preferences and complete a full assessment within fourteen days of move-in. The facility failed to obtain copies of safety assessments from the physical therapy team and had to conduct them during the inspection.
WAC 388-78A-2060 Preadmission assessment. The facility must conduct a preadmission assessment including medical history, medications, diagnosis, behaviors, personal care needs, and preferences. The facility had incomplete documentation initially but updated assessments after admission to meet requirements.

The Office of the State Fire Marshal conducted a scheduled fire safety inspection at the facility on 04/02/2025.

Findings
The facility was found unable to provide documentation for quarterly sprinkler system inspections and was required to provide forward flow testing of backflow preventers. The report was approved with these conditions noted and a follow-up inspection scheduled.

The inspection was conducted in response to a complaint regarding a fire alarm at the Fairview Assisted Living Facility.

Complaint Details
Complaint #172662 involved a fire alarm complaint. The investigation confirmed that a valve was not completely closed causing the alarm, but no violations were cited.
Findings
The inspection found that an annual fire sprinkler testing and maintenance inspection was completed and identified a valve that was not completely closed, causing an alarm. The fire department responded and cleared the system. No violations were cited and the facility was approved.
