This report covers 12 inspections of Journeys Crossing Assisted Living from April 16, 2021 to December 17, 2025, including routine, renewal, complaint, and monitoring inspections.

Complaint Details
Three complaint surveys were conducted. One on December 17, 2025 cited one violation related to staffing plan deficiencies. Two other complaint surveys on July 17, 2025 found no violations after investigation of allegations.
