Visit Reason
This inspection was conducted as an annual State Licensure survey of the facility in accordance with Nevada Administrative Code (NAC), Chapter 449, Residential Facilities for Groups.
Findings
The facility received a grade of A but had regulatory deficiencies including failure to provide direct supervision for an employee with an undetermined background check, failure to secure a pool gate, and failure to obtain an initial placement assessment for a resident.
