A March 2021 investigation cited four deficiencies: failure to provide contracted shower assistance, inadequate registered nurse supervision causing delayed treatment for a resident with urinary tract infection and dehydration, failure to notify legal representatives of medication changes (four residents), and failure to coordinate care with home health providers. June 2023 inspection found food labeling violations and unlocked medication cart left unattended. An August 2019 renewal cited medication review gaps affecting four residents, inadequate glucose and blood pressure monitoring (two residents each), and failure to report injuries requiring outside treatment to the state.
Complaint investigations in October 2022 and October 2020 found no deficiencies. No enforcement actions or license suspensions are recorded.
Families should ask about corrections following the December 2025 deficiencies, the March 2025 substantiated failures, and whether all 21 deficiencies have been fully resolved.
