The October 21, 2024, investigation substantiated verbal abuse by a staff member toward two residents, resulting in termination. That same inspection found one dietary staff member not current with food service training and four of five sampled staff lacked CPR and first aid certification.

An April 2021 investigation identified inadequate monitoring of blood thinners and failure to assess changes in condition, with immediate jeopardy initially cited but later resolved. A March 2021 investigation cited four deficiencies: failure to provide contracted shower assistance, inadequate registered nurse supervision causing delayed treatment for a resident with urinary tract infection and dehydration, failure to notify legal representatives of medication changes (four residents), and failure to coordinate care with home health providers. June 2023 inspection found food labeling violations and unlocked medication cart left unattended. An August 2019 renewal cited medication review gaps affecting four residents, inadequate glucose and blood pressure monitoring (two residents each), and failure to report injuries requiring outside treatment to the state.

Complaint investigations in October 2022 and October 2020 found no deficiencies. No enforcement actions or license suspensions are recorded.

Families should ask about corrections following the December 2025 deficiencies, the March 2025 substantiated failures, and whether all 21 deficiencies have been fully resolved.
