The facility failed to provide adequate services to tenants, complete required occupancy agreements, conduct timely cognitive assessments, and develop appropriate service plans. The facility implemented corrective actions including updates to service plans, audits, staff re-education, and monitoring.

The inspection was conducted as an initial certification visit to determine compliance with certification rules for an Assisted Living Program for People with Dementia, including investigations into multiple complaints.

Complaint Details
The inspection included investigations into Complaint #117891-C, Complaint #117574-C, and Complaint #116989-C.
Findings
The inspection identified multiple regulatory insufficiencies including failure to treat tenants with dignity, failure to provide timely responses to visitation requests, prohibiting legal representatives from administering medications, failure to administer medications as prescribed, failure to update service plans to address tenant behaviors, and failure to have a signed occupancy agreement for a tenant.
